Travel Advice & Vaccinations

To help us offer the appropriate service, you will be required to complete a travel risk assessment form.

You can download the travel risk assessment form here.

This should be submitted at least 6 weeks before travel.  Once this has been received, one of the practice nurses will contact you (usually by email) with advice.

If the travel risk assessment form is submitted with less than 3 weeks notice, we cannot guarantee an appointment for your travel vaccinations. 

NHS Health Checks

We offer NHS Health Checks to all patients aged 40 to 74 to help identify early signs of stroke, kidney disease, heart disease, type 2 diabetes, or dementia. This check is designed to assess your overall health and guide you on reducing risks.

Mental Health Reviews

Our Mental Health Reviews are conducted by our dedicated Mental Health Nurse, providing support and guidance for patients managing mental health conditions. These reviews offer a safe space to discuss symptoms, treatment options, and any concerns you may have, ensuring you receive the appropriate care and support.

Long-Term Conditions Reviews

We offer regular Long-Term Conditions Reviews to help manage and monitor ongoing health conditions, ensuring you receive the best possible care. These reviews include assessments for asthma, chronic kidney disease (CKD), COPD, coronary heart disease, dementia, diabetes, hypertension (high blood pressure), and stroke/TIA.

If you have a long-term condition, our healthcare team will invite you for a review to discuss your treatment, symptoms, and any necessary lifestyle adjustments.

Immunisations

All children’s vaccinations are offered as per the recommended national schedule as well as shingles, pneumococcal (pneumonia) and seasonal influenza vaccines.

RSV vaccine

The RSV vaccine helps protect against respiratory syncytial virus (RSV), a common virus that can make babies and older adults seriously ill. It’s recommended during pregnancy and for adults aged 75 to 79.

ECG (Electrocardiogram)

An ECG (Electrocardiogram) is a simple and painless test that records the electrical activity of your heart. It helps in diagnosing various heart conditions, such as irregular heart rhythms and other cardiac concerns.

This service is available at the practice by appointment. If your doctor has recommended an ECG, please contact the reception to schedule your test.

Ear Irrigation

Our Ear Irrigation service is available for patients experiencing a build-up of earwax that may be causing discomfort or hearing difficulties. The procedure is carried out by trained healthcare professionals using a safe and effective method to remove excess wax.

If you are experiencing blocked ears, reduced hearing, or discomfort, please contact the practice to discuss whether ear irrigation is appropriate for you.

Dispensary

Our Dispensary is available to patients residing in Upchurch who live more than 1.6km from the nearest community pharmacy. This service allows eligible patients to conveniently collect their prescribed medications directly from the practice.

Dementia Reviews

Our Dementia Reviews are provided by our dedicated Mental Health Nurse and Dementia Coordinator to support patients living with dementia and their caregivers. These reviews help assess ongoing needs, monitor symptoms, and provide guidance on managing the condition.

If you or a loved one require a dementia review, please contact the surgery to arrange an appointment.

Contraception

Our practice nurses offer confidential contraceptive advice to help you choose the best option for your needs. We provide guidance on a range of contraceptive methods, including pills, patches, injections, and other forms of birth control.
